Pharmacy Care Coordinator – Copeland

Working directly with the Surgery Based Pharmacists to ensure that patient centered coordinated care is achieved alongside NICE and safe prescribing guidelines.

The role will proactively identify the patients who require clinical intervention. The role will be delivered through contact with patients, pharmacist, medicines team, reception and all clinicians.

£22,500 – £26,413 p.a.
